Please read carefully the
Terms and Conditions of Membership (in particular
the "Personal Data" section) before completing
and submitting the application via the Internet. Applicants
must be Hong Kong residents aged 18 or over. Application
form & questionnaire must be completed in full (in
English) and with other required documents. Failure
to complete the required particulars render SHKP Club
the right not to admit you as a Member.
I understand that after completing the application
form, I have to provide a copy of HKID card and/or other
required documents by post or fax (fax no. 2827 8804).
The copy of HKID card will be destroyed after the identity
card number has been checked. Alternatively, I have
to produce physically my identity card in person for
checking and/ or submit the other required documents
at the Club's office at 52/F., Sun Hung Kai Centre,
30 Harbour Road, Hong Kong.
I confirm that I have read and understood the Terms
and Conditions of Membership and agree to be bound by
such Terms and Conditions of Membership, whereupon I
am admitted as a Member of SHKP Club. I also declare
that all information supplied by me is true, correct
and complete. I understand and agree that all documents
supplied in connection with this application will not
be returned, and upon request I shall supply original
or such other documents to facilitate your scrutiny
of this application.
